Dr. Medina Galvan is a designated FAA Aviation Medical Examiner, authorized to perform Class 1, 2, and 3 flight physicals and submit them to the FAA through MedXpress at the time of your visit.
A $100 deposit reserves your appointment and comes off your exam fee.
This is the one thing that will stop your exam at the door. Dr. Medina cannot legally perform the physical without your form on file.

The airman medical exam, also called the flight physical, verifies that a pilot’s health meets FAA standards to safely operate an aircraft. It is required for any pilot seeking or renewing a medical certificate.
Dr. Medina Galvan is FAA-designated to administer Class 1, 2, and 3 physicals and submits results directly to the FAA through MedXpress while you are still in the office.
This is an occupational screening exam. It is not a replacement for your annual preventive physical, and we are not treating medical conditions at this visit.

Assuming your 8500-8 is already filed, three things.
A valid government-issued photo ID. If this is a renewal, bring your previous medical certificate as well.
If you wear corrective lenses daily, wear them in or bring them with you for the vision screening.
Cash or a credit card, due at the visit. Your $100 deposit comes off the total. We do not file with insurance.a
Important to Know
Three of them, and the first one is the one people learn the hard way.
File it through MedXpress and email us the confirmation number and a copy before you come in. Dr. Medina cannot perform your exam without it on file, and turning up without it means turning around.
We need 24 hours’ notice to reschedule. An exam cancelled with less notice will not be rescheduled, and a $100 no-show fee applies.
These add a $50 administrative fee, and more importantly they add time. Tell us when you schedule so we book the room for long enough to do it properly.
Cash and credit cards, due at the visit. We do not bill insurance for flight physicals, and no insurer covers them anyway.
